Study Finds Lower Same-day Discharge Failure Rates in ASCs versus Hospitals after TJA
A retrospective analysis in the Journal of the AAOS® investigated perioperative risk factors of same-day discharge (SDD) failure in 466 patients after TJA. Surgeries performed in an ASC had a significantly lower rate of failed planned SDD versus hospital surgeries (0.3% versus 33.3%). The one ASC failure that occurred resulted from pain hindering physical therapy. Reasons for in-hospital SDD failure included medical complications (62%), therapy clearance issues (24%), logistical problems with nonsurgical care on day of surgery (8%), and unknown causes (6%).
Study Compares Revision Risk between THA with Dual Mobility Cups versus Highly Crosslinked Polyethylene
According to a study in the Journal of Arthroplasty, dual mobility acetabular cups (DMCs) had a similar 6-year revision risk after THA compared with unipolar metal-on-crosslinked-polyethylene (MoXLPE) and unipolar ceramic-on-XLPE (CoXLPE). There were no significant differences in aseptic revision risk for MoXLPE (hazard ratio [HR]: 1.04) or CoXLPE (HR: 0.98) compared to DMC. Similarly, no significant differences in dislocation risk were observed for MoXLPE (HR: 1.42) or CoXLPE (HR: 1.25) compared to DMC in the adjusted analyses.
BMI ≥40 Is Associated with an Increased Risk of PJI within 2 Years of TSA
A data-driven BMI threshold of 40 kg/m2 was significantly associated with an increased risk of 2-year PJI after TSA, according to a study in the Journal of Shoulder and Elbow Surgery. Stratum-specific likelihood ratio analyses were conducted to examine the relationship between BMI strata and risk of 90-day complications and PJI. Patients with a BMI ≥40 had a higher risk of 2-year PJI (risk ratio: 2.7) compared with a BMI of 19 to 39 but no difference in 90-day major complications.
Systematic Review Assesses Efficacy of Corticosteroids, Hyaluronic Acid, and PRP for Knee Osteoarthritis
BMC Musculoskeletal Disorders published a systematic review and meta-analysis that compared the efficacy of corticosteroids, hyaluronic acid (HA), platelet-rich plasma (PRP), and combination therapy for knee osteoarthritis. Thirty-five studies including 3,104 patients comprised the analysis. PRP had the best WOMAC scores at 3, 6, and 12 months. PRP plus HA scored highest in VAS at 3 and 12 months. The use of no therapy was associated with increased adverse events.
Which Treatment Is Associated with Better Outcomes for Idiopathic Clubfoot?
A meta-analysis of 11 randomized controlled trials in the Journal of Bone and Joint Surgery compared the outcomes of different interventions to treat idiopathic club foot. Based on the surface area under the cumulative ranking curve, the Ponseti method was associated with the best Pirani score changes, ankle dorsiflexion, number of casts, adverse events, and complications. The accelerated Ponseti method was best for time in casts and tenotomy rate, while early tibialis anterior tendon transfer had the lowest relapse rate.
