Study: Associations between Antibiotic Allergies and TSA Complications

The Journal of the AAOS ® published a retrospective cohort analysis analyzing the risk of postoperative complications for patients undergoing total shoulder arthroplasty (TSA) with a history of antibiotic allergy (ABX). Overall, 90-day medical complications and rate of revision at 30 days, 90 days, one year, and two years were collected from 85,606 patients undergoing TSA for osteoarthritis. Of these patients, 9.15 percent had a reported ABX. Patients with reported ABX were at an increased risk of periprosthetic joint infection, postoperative anemia, blood transfusion, and readmission 90 days postsurgery.

Study Investigates the Effect of Tibial Rotation on Medial Meniscus Posterior Horn Tears

According to a retrospective case-control study published online in Knee Surgery, Sports Traumatology, Arthroscopy, tibial rotation is significantly reduced in patients with medial meniscus posterior horn tears (MMPHTs). Tibial rotation, femoral version, tibial slope, and knee varus were measured in 50 patients with meniscal tears and 57 knees with an intact meniscus. Mean tibial rotation and mean mechanical axis were significantly different between groups, at 26.3 degrees and 3.7 degrees in the MMPHT group and 30.3 degrees and 2.05 degrees in the control group, respectively. The study suggested that internal torsion of the tibia may play a role in the etiology of MMPHTs.

Study: Postoperative Outcomes and Risk of Subsequent Surgery after Arthroscopic Posteromedial Osteophyte Resection

A retrospective cohort study published online in the Journal of Shoulder & Elbow Surgery investigated the outcomes and rates of ulnar collateral ligament reconstruction (UCLR) following arthroscopic resection of posteromedial osteophyte in overhead athletes. The primary outcomes were return to sport (RTS), surgical complications, and subsequent surgery. Of the 36 patients included in the analysis, 77 percent had successful RTS. Follow-up UCLR was required in five patients. Three UCLRs were performed within 13 months after the initial operation, while the remaining two patients underwent UCLR at 6.2 years and 7.5 years postsurgery, respectively.

Study: Preoperative Blood Loss in Patients with Isolated Acetabular Fracture

Patients undergoing operatively treated isolated acetabular fractures lost an average of one liter of blood in preoperative settings, according to a study published online in the Journal of Orthopaedic Trauma. In two level 1 trauma centers, the volume of preoperative blood loss and transfusion rates associated with isolated acetabular fracture patterns in 598 patients were retrospectively reviewed. Both column fractures and T-type fractures resulted in the greatest average preoperative blood loss (1,454.9mL and 1,374.8mL, respectively). Acetabular fracture patterns were significantly associated with preoperative blood loss and preoperative transfusion rates.

Study: Outcomes of Lateral Tibial Tension Band Plating and Guided Growth following Early Tibial Osteotomy

The Journal of Pediatric Orthopaedics published a study online evaluating the effects of lateral tibial tension band plating (LTTBP) on guided correction for varus deformity following early tibial osteotomy in infantile tibia vara (ITV) patients. Fifteen patients were included in the study and correction of deformity parameters on digital standing anteroposterior lower extremity radiographs were measured. Single-event LLTBP improved the medial proximal tibial angle a mean of 13.4 degrees. After initial osteotomy in ITV patients, LTTBP for residual varus resulted in 81 percent of limbs achieving mechanical axis zone I or valgus with implant failure revisions and femoral remodeling.
