Study: High Risk of Revision THA and TKA Surgery in Multiple Myeloma Patients Treated with Radiotherapy
Patients with multiple myeloma treated with radiotherapy had a relatively high risk of requiring revision total hip (THA) and knee arthroplasty (TKA), according to a study in the Journal of Arthroplasty. Examining the one-year postoperative outcomes in 78 THA and 26 TKA patients with multiple myeloma, demographic data, oncologic treatments, and operative variables were collected and analyzed. Overall, 11.5 percent of THA patients underwent revision surgery on average 1,312 days postsurgery, and 3.8 percent of TKA patients underwent revision after 74 days. The most common indications for revision surgery were infection (33.3 percent), periprosthetic fracture (22.2 percent), and instability (22.2 percent).

Study Examines Effect of Vitamin D Deficiency on Patellar Instability
A study in Sports Health: A Multidisciplinary Approach investigated the association between vitamin D deficiency and patellar instability. The study was a 1:1 matched retrospective study of 328,011 patients diagnosed with vitamin D deficiency. Rates of primary patellar instability and surgical stabilization for recurrent dislocation were calculated based on sex- and age-specific stratifications. In patients with vitamin D deficiency, the overall one-year incidence rate of patellar instability was 82.6 per 100,000 person-years compared with 48.5 in the matched control group. Younger patients (10 to 25 years) with hypovitaminosis D were more likely to experience recurrent patellar stabilization for both men and women.

PRP Injections for Ankle OA Show Short-term Improved Pain and Functional Scores
The Journal of Orthopaedic Surgery and Research published a systematic review and meta-analysis that pooled individual studies in order to assess the efficacy of platelet-rich plasma (PRP) injections for ankle osteoarthritis (OA). Five studies, including one randomized controlled trial and four before-after studies, were analyzed. PRP was associated with significantly reduced visual analog scale scores and improved functional outcomes in ankle OA patients at 12 weeks.

Study Compares Distal Spine versus Pelvic Anchors in Pediatric Patients with Hypotonic Neuromuscular Scoliosis
A retrospective review in the Journal of Pediatric Orthopaedics sought to identify indications to exclude pelvic fixation in pediatric patients with hypotonic neuromuscular scoliosis (NMS) treated with growth-friendly instrumentation. The study included 38 patients who had distal spine anchors (DSAs) and 87 who had distal pelvic anchors (DPAs) placed at the index surgery. DSA patients had a lower pelvic obliquity (11 versus 19 degrees) and L5 tilt (8 versus 12 degrees) in pre-index radiographs, and post-index radiographic data were comparable between the groups. Additionally, no differences were observed regarding the rates of complication and unplanned returns to the OR.

Updated CPG Offers Recommendations for Surgical Management of Knee Osteoarthritis
The AAOS Clinical Practice Guideline (CPG), approved by the Board of Directors at the end of 2022, updates the original 2015 document on Surgical Management of Osteoarthritis of the Knee (SMOAK) with revisits and revisions for 19 of 38 evidence-based recommendations. “The most noteworthy aspect of this CPG is the incorporation of the most up-to-date, high-level evidence that’s available in our literature to guide decision-making with respect to the surgical management of knee arthritis,” Jonathan Godin, MD, FAAOS, co-chair of the SMOAK guideline work group stated.
