Study Reports Low Rate of ED Utilization after Outpatient Rotator Cuff Repair

A retrospective study published in the Journal of the AAOS ® reported low rates of ED utilization after outpatient elective rotator cuff repair. Cases from 2014 through 2015 from New York and Florida state databases were included. Rates of seven- and 30-day all-cause ED use were 3.2 percent and 5.0 percent, respectively. Pain and gastrointestinal complaints were the most common cause of seven-day and 30-day ED visits, respectively. African American or Hispanic race/ethnicity and comorbid hypertension, diabetes, and schizophrenia were independent risk factors for ED use.

Study: Preoperative Tramadol Use Is Associated with Increased Opioid Use after TJA

A retrospective study published online in The Journal of Arthroplasty reported that preoperative tramadol use before total joint arthroplasty (TJA) was associated with increased postoperative opioid use. In total, 11,667 patients from a single center were included. Eleven percent of patients reported preoperative tramadol use, and 6.3 percent reported narcotics and tramadol use. Compared to opioid-naïve patients, preoperative tramadol use was associated with significantly higher in-hospital morphine milligram equivalents (MMEs), post-discharge MMEs, and opioid refill rates.

Study: Incidence of Hand Trauma at Community-based Emergency Centers

A study published online in Injury investigated the rates of hand trauma at community-based emergency centers (CBEC) compared to hospital EDs. All hand injury cases from a chain of CBECs in 2017 were assessed (n = 53,574), then compared to published data on hand trauma in EDs. Rates of hand injuries were higher in CBECs than EDs (35 percent versus 20 percent). Eighty percent of hand injuries in the CBEC setting were fractures and 10 percent were lacerations. Older patients were treated less frequently with immobilization than younger patients.

Study Reports Rising Incidence of Surgery for Pediatric Middle Shaft Clavicle Fractures

A single-center study published online in BMC Musculoskeletal Disorders reported an increasing incidence of surgical fixation for pediatric middle shaft clavicle fractures since 2008. In total, 172 patients aged <16 years treated between 2008 to 2019 were included. Surgical management increased from zero cases per 100,000 age-adjusted children in 2008 to 10.8 per 100,000 children in 2019. Incidence of surgery rose from 2.6 percent from 2014 to 2016 to 16.1 percent from 2017 to 2019. Displacement of >15 mm, shortening of >15 mm, and age >9 years were associated with increased risk of surgery.
