Study Assesses Trends in Use of Open Bankart Procedure Over Time

A study published in the March 15 issue of the Journal of the AAOS ® surveyed members of the American Shoulder and Elbow Surgeons regarding their experiences with the open Bankart repair procedure in residency, fellowship, and practice. Responses to the 10-question survey were divided based on year of completion of training. There was a notable trend of progressively reduced use of the open Bankart by surgeons trained prior to 2000 compared to those trained after 2001. The technique was increasingly used for revision rather than primary procedures over time.

Study: Utility and Impact of Cardiology Evaluation and TTE on Time to Surgery for Geriatric Hip Fracture

A retrospective study published in the April issue of the Journal of Orthopaedic Trauma evaluated delays in time to geriatric hip fracture surgery from preoperative cardiology consultation and transthoracic echocardiogram (TTE) among 412 patients. Overall, 17.7 percent met American Heart Association and American College of Cardiology preoperative consultation criteria, although 44.4 percent received consultation. TTE criteria was met in 33.8 percent, and 89.4 percent received TTE. Consultations increased time to surgery compared to patients without consultation (25.42 hours versus 19.27 hours). TTE also increased time to surgery compared to patients who did not receive one (26.00 hours versus 18.94 hours)

Study Evaluates Causes of ED Visits after TKA

A study published online in The Journal of Arthroplasty assessed the incidence and risk factors of emergency department (ED) visits following total knee arthroplasty (TKA). Using a nationwide database, 1,364,655 patients who underwent TKA were identified, of whom 5,689 had an ED visit within 90 days postoperatively. Patients who had ED visits had higher mean Elixhauser Comorbidity Index scores compared to those without visits (nine versus six). Musculoskeletal etiologies were the most common cause of ED visits. Other comorbid conditions associated with ED visits were hypertension, peripheral vascular disease, coagulopathy, and rheumatoid arthritis.

Study: Impact of Sleep Disturbance on Quality of Life Following Spine Surgery

A retrospective study published online in The Spine Journal measured the incidence and impact on health-related quality of life of sleep disturbance after spine surgery. In total, 508 adults who received spine surgery between 2014 and 2018 at a single center were included. A quarter of patients (n = 127) had self-reported preoperative sleep disturbance, which resolved in 63 percent of patients postoperatively. Patients with ongoing sleep disturbance were less likely to show postoperative improvements in pain, physical function, and postoperative satisfaction. Sleep disturbance was associated with significantly lower Oswestry Disability and Neck Disability Indices.

Study: Outcomes of Aseptic Kirschner Wire Pinning for Hand and Wrist Fracture

A retrospective study published online in Injury evaluated outcomes of an aseptic technique for percutaneous Kirschner wire (K-wire) pinning for hand and wrist fracture repair. For this technique, K-wires were not buried or bent and covered with betadine-soaked alcohol pads as pin site dressings. After 12 weeks, pins were removed via outpatient procedure. Ninety patients receiving a variety of K-wire fixation procedures were included. No cases of infection were reported, as well as no other complications. No patients required postoperative antibiotics, early pin removal, or reoperation.
