Retrospective Analysis Evaluates Factors Influencing Postoperative Femoral Head Collapse in Patients with Aseptic Necrosis

A retrospective analysis of the factors influencing postoperative femoral head collapse (FHC) in patients with Ficat I, II, and III stages of aseptic necrosis of the femoral head (ANFH) was published in the Dec. 15 issue of the Journal of the AAOS ®. Patients with ANFH (n = 178) were categorized into the FHC group and no FHC group based on whether FHC was observed postsurgery. Hormonal osteonecrosis of the femur, extent of necrosis, and distance of the tip of the tantalum rod from the center of necrosis were risk factors for postoperative FHC.

Study Compares Liposomal Bupivacaine and Continuous Nerve Block for Pain Management after TKA

The use of liposomal bupivacaine (LB) for pain management following total knee arthroplasty (TKA) was compared with continuous nerve block with a pain pump in a retrospective study in The Journal of Arthroplasty. Average pain scores, 30- and 90-day readmission, and ED visits were collected in 1,640 patients treated with LB and 738 treated with a pain pump. Milligram morphine equivalents (MMEs) per admission, length of stay (LOS), and total costs were secondary outcomes. There were no significant differences regarding the primary outcome measurements, MMEs, or LOS. A potential cost saving was estimated to be $95 per LB patient.

Retrospective Review Finds Iron Deficiency Anemia Is Associated with Increased Complications and LOS after TSA

Patients undergoing total shoulder arthroplasty (TSA) with iron deficiency anemia (IDA) had longer in-hospital lengths of stay (LOS), according to a study in the Journal Shoulder and Elbow Surgery. Overall, 17,689 patients with IDA and 88,445 without IDA comprised the matched-cohort population. The main outcomes included LOS and 90-day medical and implant-related complications postsurgery. In addition to longer LOS (3 days versus 2 days), IDA patients had significantly higher odds of 90-day medical and implant-related complications, which included blood transfusions and periprosthetic joint infections.

Cross-sectional Study Investigates Association between Physeal Status and ACL Tear in Skeletally Immature Patients

The Journal of Pediatric Orthopaedics published cross-sectional study investigating whether physeal status had an influence on the likelihood of sustaining an ACL tear when a classic bipolar ACL bone bruising pattern was present in skeletally immature patients. The main outcome was the incidence of ACL tear in patients with bipolar bone contusions. Of the 499 patients included in the study, 269 had bipolar bone contusions. The duration between injury and imaging date was shorter in patients with bipolar bone contusions (6.9 days versus 38.6 days). Patients who had an open femoral physis were more likely to have an intact ACL.
