Study: Length of Stay and Coding Practice Changes after 2018 Removal of TKA from IPO List

The 2018 removal of total knee arthroplasty (TKA) procedures from Medicare’s inpatient only (IPO) list significantly impacted discharge location and coding practices in TKA, according to a study published in the Dec. 15 issue of the Journal of the AAOS ®. The National Surgical Quality Improvement Program database was queried for TKAs from 2015 to 2018. Average length of stay decreased significantly from 2017 to 2018 (2.31 days to 2.05 days), and incidence of same-day discharge increased (2.28 percent to 5.09 percent). Patient coding changed significantly as well: in 2017, the rate of “outpatient” coding was 1.66 percent, compared with 17.14 percent in 2018.

Study: Leukocyte Esterase Strip Test at Reimplantation Reliably Predicts Persistent PJI

A retrospective study of 76 patients published online in The Journal of Arthroplasty found that a leukocyte esterase (LE) strip test reliably diagnosed persistent periprosthetic joint infection (PJI) and outperformed serum assays. Data on 76 patients undergoing reimplantation for PJI were analyzed. Other evaluated assays included serum C-reactive protein (CRP), erythrocyte sedimentation rate (ESR), or a combination thereof. The sensitivity, specificity, and positive and negative predictive values of the LE assay were 82 percent, 99 percent, and 90 percent and 97 percent, respectively, compared with 82 percent sensitivity and 84 percent specificity for CRP and 55 percent sensitivity and 87 percent specificity for ESR.

Randomized Trial Finds Benefit of Multimodal Nonopioid Protocol after Arthroscopic RCR

A randomized clinical trial of 40 patients, published online in Arthroscopy, found that a multimodal nonopioid pain protocol after arthroscopic rotator cuff repair (RCR) provided effective pain control compared to traditional opioid analgesics. When controlling for confounders, the nonopioid group (n = 17) demonstrated significantly lower Visual Analog Scale pain scores and patient-reported pain interference scores during the first 10 postoperative days compared with the opioid group (n = 23). The opioid group also had prolonged gastrointestinal side effects compared with the nonopioid group.

Survey Study Assesses Patient Acceptability of Costs for Hand Surgery

A study published in the Journal of Bone & Joint Surgery surveyed 1,408 individuals regarding willingness to pay out-of-pocket prices for three hand surgery procedures: carpal tunnel release, cubital tunnel release, and distal radial fracture fixation. Respondents were randomized to one of five price options for each procedure. Nearly 80 percent reported willingness to pay for all procedures regardless of price. Price sensitivity was highest for carpal tunnel release, with 17 percent rejecting surgery at the highest price ($3,000) and 6 percent rejecting at the lowest price ($250). The use of older-generation implants was the least accepted cost-cutting measure, with a 50 percent acceptance rate.

New CPG Updates Guidance on Non-arthroplasty Management of Knee OA

At its August meeting, the AAOS Board of Directors approved the 2021 Clinical Practice Guideline (CPG) on Treatment of Osteoarthritis (OA) of the Knee (Non-arthroplasty) Third Edition. The CPG contains updates to all 19 evidence-based recommendations included in the second edition of the guideline, with a total of 29 non-arthroplasty treatment recommendations. This article reviews the updates, with insight from Robert H. Brophy, MD, FAAOS, cochair of the development group for this CPG.
