Study Finds Increased Risk of Lateral Femoral Cutaneous Nerve Injury in THA Patients with History of Hip Arthroscopy

Arthroscopy, Sports Medicine, and Rehabilitation published a retrospective study investigating the rates of lateral femoral cutaneous nerve (LFCN) injury in patients with and without previous hip arthroscopy. A total of 179 total hip arthroscopy (THA) patients treated via a direct anterior approach were included. Of these, 166 had no history of arthroscopy and 13 patients did. The incidence of LFCN injury was compared between groups. Overall, 77 patients had some form of LFCN injury six-weeks postsurgery. The rate of LCFN injury was significant higher in patients with a history of arthroscopy at six weeks following surgery (39 percent versus 92 percent).

Nationwide Analysis Examines Characteristics of Orthopaedic Medical Malpractice Claims

According to a nationwide database study in The Journal of Arthroplasty, hip and knee cases had the highest incidence in orthopaedic malpractice litigation. Examining all orthopaedic medical malpractice claims from 2015 to 2020, 164 claims from 17 states were identified. The variables of interest included case outcome, indemnity payment, damages, negligence claimed, treatment, and patient characteristics. Of the hip and knee cases, 77.6 percent resulted in defense verdicts, 18.4 percent resulted in plaintiff verdicts, and 4.1 percent resulted in settlements. Paralysis, amputation, and death were associated with a decreased likelihood of a defense outcome.

Study Assesses Association between Limb Alignment and Lower Extremity Rotation in Patients with Patellar Instability

A cross-sectional study found that tibial torsion was associated with leg axis alignment and femoral anteversion in patients with patellar instability. The study in the Orthopaedic Journal of Sports Medicine investigated the relationship between lower limb alignment and lower extremity rotation among 83 patients. Tibial tuberosity–trochlear groove (TT-TG) distance and mechanical femorotibial angle (mFTA) were measured via standing full-leg radiographs. There were no significant correlations between mFTA and TT-TG distance or femoral anteversion; however, patients with varus malalignment had higher tibial torsion compared with patients with valgus malalignment.

Retrospective Study Investigates Changes in the Medial Longitudinal Arch Height of the Foot after Ankle Arthrodesis

International Orthopaedics published a retrospective study examining the changes in medial longitudinal arch height of the foot after ankle arthrodesis in patients with severe ankle arthritis. Thirty-seven patients with a preoperative tibial axis to talar dome angle of <80 degrees were included in the analysis. Patients were subsequently divided into two groups according to postoperative decreases in medial cuneiform height (MCH). Sixteen patients had a postoperative MCH decrease of >2mm. The degree of coronal ankle varus correction was similar between groups; however, patients with an MCH decrease of >2mm had a significantly greater decrease in the Meary angle.

Gold Card Legislation Can Aid in Prior Authorization Reform

For this installment of Advocacy 201, Douglas W. Lundy, MD, MBA, FAAOS, chair of the AAOS Advocacy Council, discusses federal and state-level efforts in prior authorization reform—such as gold card legislation, which “allows physicians that have high prior authorization approvals to bypass these requirements for a certain period,” Dr. Lundy wrote. According to a 2020 report on gold card legislation in Vermont, the gold card program successfully improved patients’ clinical results and reduced the administrative burden of healthcare professionals who used it for Medicaid radiology services.
