Randomized Trial Assesses Outcomes of Three Commonly Used Patellar Implants in TKA

A randomized trial published online in the Journal of Bone & Joint Surgery compared the outcomes of inlay (IN), onlay round (OR), and onlay oval (OO) patellar implants in total knee arthroplasty (TKA). Kujala scores and clinical, intraoperative, and radiographic outcomes were measured pre- and two years postsurgery. There were 40 patients in the IN group, 41 in the OR group, and 40 in the OO group. No significant differences were noted between groups regarding Kujala scores. Patients with OO implants demonstrated improved superior bone coverage and improvements in several intraoperative and radiographic outcomes.

Study Investigates Association between Preoperative VR-12 Scores and Healthcare Utilization after THA

According to a study in The Journal of Arthroplasty, preoperative Veterans RAND 12-Item Health Survey (VR-12) mental component scores (MCS) of <40 were associated with increased healthcare utilization following total hip arthroplasty (THA). VR-12 MCS, 90-day hospital resource utilization, and attainment of a patient acceptable symptom state (PASS) one-year postoperatively were measured in a prospective cohort of 4,194 patients. Preoperative VR-12 MCS <20 was associated with more than double the odds of non-home discharge and prolonged lengths of stay. VR-12 MCS >60 was associated with higher odds of reaching PASS.

Systematic Review Finds PRP Is Associated with Reduced Retear Rates in Surgically-treated Rotator Cuff Tears

Knee Surgery, Sports Traumatology, Arthroscopy published a systematic review assessing the clinical efficacy of platelet-rich plasma (PRP) injections in patients with rotator cuff disorders, treated either surgically or nonsurgically. A meta-analysis included 36 randomized controlled trials and 2,443 patients, with data on visual analog scale (VAS) scores, simple shoulder test (SST) outcomes, and retear rate. There were no clear advantages to PRP augmentation in conservatively-treated patients. There were no significant differences noted regarding VAS and SST scores in surgical treatment studies. However, the rate of retear was lower in rotator cuff surgical repair patients with PRP injections.

Retrospective Study Evaluates the Effects of Cage Positioning and Height on Outcomes in Patients Undergoing Lateral Lumbar Interbody Fusion

In a retrospective study in BMC Musculoskeletal Disorders, cages in anterior positions showed better restoration of disk height (DH) and segmental angle (SA) and decreased cage subsidence in patients undergoing lateral lumbar interbody fusion (LLIF). Researchers investigated the radiographic and clinical outcomes in 47 patients with a mean follow-up of 12.8 months. Cage height and position did not result in significantly improved clinical outcomes. Improvement in degrees of DH, SA, and lumbar lordosis was greater with cages of 11mm compared with 13mm cages.

The ROCK is rolling. The AAOS Resident Orthopaedic Core Knowledge program—“the ROCK”—is now nearing completion. Content in several subspecialties is currently online and in use by residents and program directors, and the remaining content areas should be in place by the end of February 2023.
