Strong Correlation Found between Patient-Reported and Clinician-Measured Shoulder ROM

 

A retrospective study in the Journal of the AAOS® evaluated the correlation between patient-reported (PR) and clinician-measured (CM) shoulder range of motion (ROM) using data from 13,842 visits over 16 years. There were strong correlations between CM and PR elevation and internal rotation and a moderate correlation between CM and PR abduction. Elevation, internal rotation, and abduction showed strong correlations in an arthroplasty subgroup analysis.

Study: Younger Patients with PCL Injuries from High-Energy Trauma Have Higher Complication Rates

 

The Journal of Orthopaedic Surgery and Research published a retrospective study analyzing the complications and associated injuries of 416 patients who underwent posterior cruciate ligament (PCL) reconstruction due to high-energy trauma. PCL injuries often involved multiple ligaments (76.7%) and were frequently treated with single-stage surgeries (86.8%). Car accidents more commonly resulted in staged surgeries. Patients aged ≤18 years and staged surgeries had higher complication rates.

Study Compares Operative versus Nonoperative Management of Fragility Pelvic Ring Fractures in Older Patients

 

According to a study in the Journal of Orthopaedic Trauma, percutaneous fixation led to similar pain control postoperatively compared with nonoperative care in patients aged ≥60 years with isolated LC1 pelvic ring fractures. Patients in the operative group had more pain on admission, and patients who received nonoperative treatment had a shorter hospital stay (4.8 versus 10.6 days) and were older (81.5 versus 74.2 years). Both groups had similar 90-day mortality and readmission rates, but the operative group showed greater pain improvement after treatment.

Large Language Models Show Varied Concordance with AAOS CPGs for Rotator Cuff, ACL Injury Management

 

Arthroscopy published a study assessing the extent to which large language models (LLMs) align with AAOS Clinical Practice Guidelines (CPGs) for rotator cuff and ACL injuries. Out of 192 recommendations, 70.3% were concordant, 22.4% indeterminate, and 7.3% discordant. ChatGPT-4 showed the highest concordance, while Mistral-7B had the most indeterminate responses. No significant differences in overall concordance among LLMs were found, and only 10.4% of recommendations included detailed references.

3D Fluoroscopy Reduces Imaging Time, Length of Stay versus CT/MRI in Developmental Dysplasia of the Hip Surgery

 

A study in the Journal of Pediatric Orthopaedics compared intraoperative 3D fluoroscopy to CT/MRI for assessing hip reduction after open reduction in developmental dysplasia of the hip. Compared with CT/MRI, 3D fluoroscopy was associated with shorter imaging times, fewer cast changes, and a reduced length of stay (1.72 versus 2.20 days). There were no significant differences in further imaging or outcomes related to subluxation or redislocation.
