Study: Increased TJA Complications for Malnourished Patients with Insulin-dependent Diabetes

A study published in the Aug. 1 Issue of the Journal of the AAOS ® suggests screening total joint arthroplasty (TJA) patients with insulin-dependent diabetes mellitus (IDDM) for preoperative malnutrition. Patients were identified via the National Surgical Quality Improvement Program database. IDDM was associated with an increased incidence of malnutrition compared to patients without insulin-dependence (11.9 percent versus 9.9 percent). Patients with IDDM, but without morbid obesity had higher risk of malnourishment than morbidly obese patients without IDDM (9.9 percent versus 6.4 percent). Malnourished patients with IDDM were at highest risk of postoperative complications and infection.

Study: Patient-related Risk Factors of Infection after ACL Reconstruction

A study published online in Arthroscopy found several patient-related risk factors for infection after anterior cruciate ligament (ACL) reconstruction. Significant independent risk factors were male sex, obesity/morbid obesity, tobacco use, age <40 years, depression, opioid disorder, and concomitant simple or complex cartilage surgery. Data from 217,541 ACL reconstruction cases identified via the PearlDiver database were assessed. There were 1,779 postoperative infections (0.8 percent) within six months. Concomitant partial meniscectomy and meniscal repair were not associated with infection.

Study: Systematic Review of Conservative versus Accelerated Rehabilitation after RCR

Re-tear rates are comparable for conservative versus accelerated rehabilitation following rotator cuff repair (RCR), according to a systematic review published online in BMC Musculoskeletal Disorders. Sixteen studies (n = 1,424) were included (accelerated, n = 732; conservative, n = 692). Mean follow-up was 12.5 months. Compared to the conservative group, the accelerated group demonstrated superior external rotation at three and six months, abduction at six months, and forward elevation at six and 12 months. There were no significant differences between groups at 24 months.

Study: Patient Education Decreases Opioid Use after Soft Tissue Surgery Distal to the Wrist

An opioid education program before minor soft tissue surgery distal to the wrist significantly reduced postoperative opioid use, according to results from a randomized study published online in the Journal of Hand Surgery. In total, 174 patients were enrolled and received education (n = 90) or standard care (n = 84). The education group received preoperative education and a reference card on pain management The education group used significantly fewer opioid pills (median, 0.0; range, 0.0–13.0) compared to standard care (0.5; 0.0–40.0). One-week pain scores and number of pills prescribed predicted higher opioid use.

Outpatient Total Hip Arthroplasty Trends Continue to Shift

Total hip arthroplasty (THA) has seen a shift in recent decades toward shorter inpatient stays, including outpatient procedures in selected patients. This shift has been driven in part by advances in perioperative care and surgical techniques, but challenges remain, including identifying appropriate candidates for outpatient THA and addressing postoperative complications. This article examines the state of outpatient THA compared to traditional inpatient THA and directions for expansion and improvement.
