Study: No Negative Impact on Antibiotic Resistance after THA and TKA/UKA with Second-line Antibiotic Agents
The Journal of Arthroplasty published a study comparing bacterial causes and antibiotic resistance in patients with periprosthetic joint infection (PJI) undergoing total hip arthroplasty (THA), and total knee and unicompartmental knee arthroplasty (TKA/UKA). Cefuroxime was the standard preoperative prophylactic antibiotic for primary joint arthroplasty, with clindamycin as the second-line agent. In 3,123 cefuroxime-administered THAs, 61 had culture-positive PJI, compared with six PJIs in 206 non-cefuroxime THAs. In TKA/UKAs, 21 out of 2,455 cefuroxime cases had culture-positive PJI, while three PJIs were observed in 211 non-cefuroxime cases. No significant variations in the pathogen spectrum were observed based on the preoperative antibiotic regimen employed.

| Study Examines Factors Associated with Nonmedical Delays in Femur, Pelvis, and Acetabular Fracture Fixation
According to a retrospective study in the Journal of Orthopaedic Trauma of 313 procedures for either femur, pelvis, and acetabular fractures, nonmedical delays at an institution with a dedicated orthopaedic trauma room (DOTR) were most likely to be due to the DOTR being at-capacity and nonpelvis trauma specialists taking weekend call. Overall, 12.5 percent of procedures experienced delays, of which, 4.8 percent were attributed to nonmedical-related reasons. Nonmedical delays were more likely to be attributed to younger patients, high-energy mechanisms, Thursday through Saturday hospital admission, pelvis/acetabular fractures, and external fixation.

| Does Lumbosacral Fusion Increase Hip Joint Space?
The Journal of Orthopaedic Surgery and Research published a retrospective study which examined the effect of lumbosacral fusion on hip joint space. The sacral slope, sagittal vertical axis, pelvic incidence, lumbar lordosis, and pelvic tilt were compared in patients who underwent short-segment (n = 55) versus long-segment fusions (n = 46). No significant differences were observed between groups in terms of spinopelvic parameters and joint space narrowing, although a positive correlation was found between segment length and hip joint space narrowing.
