Does Choice of Thromboprophylactic Agent Affect Risk of VTE after THA and TKA?

 

JAMA Network Open published a retrospective study that assessed risk factors for postoperative VTE and bleeding in patients undergoing THA and TKA, comparing outcomes among different thromboprophylaxis agents. Analyzing data from a large healthcare claims database, the study found that patient-specific risk factors, rather than the choice of aspirin or direct oral anticoagulant, were associated with postoperative VTE. However, postoperative bleeding rates were lower with aspirin than other thromboprophylaxis agents.

Study Shows Comparable Outcomes in Anatomic versus Reverse TSA after Prior Contralateral Anatomic TSA

 

A review of prospectively collected data in International Orthopaedics compared outcomes between bilateral anatomic TSA (aTSA) and aTSA/reverse TSA (rTSA) for rotator cuff–intact glenohumeral osteoarthritis. Among 55 bilateral TSA patients with intact rotator cuffs, 46 had aTSA/aTSA and 9 had aTSA/rTSA. Patients who underwent aTSA/aTSA had a shorter time to second TSA; however, aTSA/rTSA patients had superior postoperative outcome scores, overhead motion, and active abduction. No differences in complications were observed between groups.

Systematic Review Compares Complications Associated with Screw Fixation during Capitellum Fracture Fixation

 

The Journal of Hand Surgery published a systematic review and meta-analysis that analyzed 42 studies to examine complication rates after surgical fixation of an acute capitellum fracture and compared the outcomes between anterior-to-posterior (A-P) versus posterior-to-anterior (P-A) screw insertion. Elbow pain was the most reported complication following fixation (21%). Avascular necrosis was higher with P-A screw insertion, while revision fixation and heterotopic ossification rates were higher with A-P insertion.

Study Examines 20-year HRQoL in Patients with Congenital or Juvenile Idiopathic Scoliosis

 

A comparative cohort study in the Spine Journal assessed the long-term health-related quality of life (HRQoL) in children with congenital scoliosis (CS, n = 29) or juvenile idiopathic scoliosis (JIS, n = 85) over a minimum of 20 years. Patients with CS and JIS showed lower vitality (CS: 60.6, JIS: 58.1) and mental health (CS: 70.0, JIS: 72.1) scores on the SF-36 questionnaire compared with the general population. No significant differences were observed in other domains.

Decompression with Vertebroplasty Results in Reduced Intraoperative Bleeding in Patients with Aggressive Vertebral Hemangiomas

 

Partial tumor resection plus decompression with vertebroplasty (VP) was associated with a more significant reduction in blood loss than decompression without VP in patients with aggressive vertebral hemangiomas and neurological deficits, according to a study in the Journal of Bone Oncology. Additionally, decompression surgery plus VP significantly reduced operation time compared with decompression surgery alone. All patients experienced immediate pain relief and neurological improvement. Recurrence occurred in only 7% of cases.

The Orthopaedic Research and Education Foundation (OREF)/AAOS Clinical Gaps Research Fund is soliciting investigator-initiated research proposals focused on the broad area of injectable orthobiologics for the treatment of knee osteoarthritis. The $50,000 grant is the fourth to be awarded through a strategic partnership between OREF and AAOS, which aims to help narrow the gap between clinical research funding and prioritize clinical research needs in musculoskeletal care, including orthobiologics. The deadline to apply is Feb. 8, 2024.
