Study Investigates Association between Initial Prescription Size and Opioid Refill following THA and TKA

The Journal of Arthroplasty published a study that found that larger initial opioid prescription sizes were associated with small but clinically insignificant decreases in 30-day refills after total knee (TKA) and hip arthroplasty (THA). Overall, 120,889 opioid-naïve patients were included in the study. The main outcome was refill of any prescription opioid medication 30 days after discharge. The 30-day refill rate was 59.6 percent after TKA and 26.1 percent after THA. The odds of refill decreased by 2 percent and 3 percent for every 75 oral morphine equivalents increase to initial prescription for the THA and TKA cohorts, respectively.

Study Evaluates Standard Methods of Heterotopic Ossification Prevention

A retrospective chart review published in Current Orthopaedic Practice investigated the efficacy of radiation and NSAID use (indomethacin) as heterotopic ossification (HO) prophylaxis after acetabular fracture surgical repair. In total, 117 patients were grouped based on the postoperative therapy received: radiation therapy, indomethacin, or no prophylaxis. Grade of HO was determined via pelvic radiograph Patients in the radiation group had a lower incidence of severe and total HO compared to the no-prophylaxis group, and the indomethacin group had significantly less total HO than the no-prophylaxis group.

Study: Effect of Bone Marrow Aspirate Concentrate and Platelet-rich Plasma on the Rates of Revision RCR

The Orthopaedic Journal of Sports Medicine published a cohort study examining the association of bone marrow aspirate concentrate (BMAC) and platelet-rich plasma (PRP) during arthroscopic rotator cuff repair (RCR) and rates of revision. Utilizing the Mariner data set from the PearlDiver database, 760 patients who underwent biologic augmentation during RCR were identified and compared with 3,800 matched controls. Patients who received BMAC at the time of RCR had significantly lower rates of revision surgery after two years. No significant differences in revision were observed between the control and PRP group.

Study: Rates and Predictors for Failure after Surgery for Lumbar Spinal Stenosis

In an observational study published in The Spine Journal, 33 percent of patients undergoing surgery for lumbar spinal stenosis reported treatment failure. In total, 8,919 patients were included in the analysis. The main outcome measurement was the Oswestry Disability Index (ODI) reported 12 months after surgery, and failure (ODI >31) and worsening (ODI >39) were used as dependent variables. Twelve months postsurgery, the mean ODI score was 23.9. Overall, 2,950 patients experienced treatment failure and 1,921 experienced worsening. The strongest predictors for failure were long duration of symptoms, past surgeries, and age >70 years.
