AAOS Comments on Proposed Medicare Payment Policy Changes for 2023
AAOS recently issued formal comments to the Centers for Medicare & Medicaid Services (CMS) on the agency’s proposed payment policy changes for calendar year (CY) 2023, including changes to the Outpatient Prospective Payment System/Ambulatory Surgical Center (OPPS/ASC) payment systems and the Physician Fee Schedule (PFS). AAOS is urging the agency to address growing healthcare costs, expand access to care, and ease physicians’ burden as they continue to navigate patient care and financial and practice management challenges exacerbated by the COVID-19 pandemic. AAOS is also asking that CMS apply recommended value changes to global codes and avoid additional mandates or pre-authorizations when surgeons are determining setting of care.

Study: Patient and Implant Factors Affecting Trunnionosis Severity
The Journal of Arthroplasty published an implant retrieval study online investigating patient and device factors associated with corrosion damage on the femoral stem taper across various implant models retrieved following revision total hip arthroplasty. Patient and device information was collected from 664 patients, and trunnions were imaged under digital microscopy. Elevated corrosion classes were associated with greater duration of implantation and larger head femoral head seize. Decreased corrosion class was associated with older age at index surgery, stainless steel stem material, indication for revision as bone or periprosthetic fracture, and infection

| Study: Functional Outcomes of Proximal Fibular Osteotomy in Knee Osteoarthritis
Proximal fibular osteotomy was found to improve the overall alignment of the joint with significant relief from pain, according to a study published online in Current Orthopaedic Practice. Sixty patients with diagnosed osteoarthritis of the knee were enrolled. Pre- and postoperative functional scores included the visual analog scale (VAS) and Knee Society Score, and radiographic parameters were measured at two weeks and three, six, and 12 months. VAS scores improved from 8.65 to 2.30 at 12-month follow-up. However, no significant improvements were seen in medial joint space and lateral joint space.

| Study: Accuracy of Reduction and Early Functional Outcomes of Anterior Approach for Fixation of Acetabular Fractures
A retrospective study, published online in the Journal of Orthopaedic Trauma, evaluated a new plate system using the anterior approach for the management of acetabular fractures. Postoperative reduction, measured by postoperative radiographs and CT, was the main outcome measurement. A total of 33 patients with a mean age of 57 years underwent reconstruction with the anatomical plates using the anterior approach. Anatomic reduction was seen in 82 percent on radiographs and CT scans. Increasing age was significantly associated with obtaining anatomical reduction. Overall, 22 patients were symptom-free, and 20 patients had excellent radiographic outcomes at the final follow-up.

| AAOS Advocacy Action Alert: Urge Congress to Vote Yes on Prior Authorization Reform Bill
This Wednesday, Sept. 14, the U.S. House of Representatives is scheduled to vote on the AAOS-endorsed Improving Seniors’ Timely Access to Care Act (HR. 8487). The bill would streamline prior authorization processes within Medicare Advantage plans and has been a key ask of legislators during events such as Orthopaedic Advocacy Week in August and the ongoing combined National Orthopaedic Leadership Conference (NOLC)/Fall Meeting in Washington, D.C. More than 65 percent of Congress now supports the bill, but AAOS members must continue urging for final passage.
