Study Investigates Improvement and Function after RSA with Prior RCR

A study published in the Feb. 1 issue of the Journal of the AAOS ® found that reverse shoulder arthroplasty (RSA) after prior rotator cuff repair (RCR) improved pain and function in patients with glenohumeral osteoarthritis or rotator cuff tear arthropathy; however, these patients had reduced postoperative function compared with patients without prior RCR. In total, 438 RSAs with prior RCR were matched to 876 controls without RCR. Prior RCR was associated with inferior postoperative forward elevation, abduction, and Constant scores compared with controls. Rates of complications and revision were similar between groups.

Study: Time to Reimplantation of >18 Weeks after Explant for PJI Is Associated with Treatment Failure

A retrospective study published online in The Journal of Arthroplasty reported that a time to reimplantation (TTR) of more than 18 weeks following two-stage resection arthroplasty for prosthetic joint infection (PJI) was associated with a higher odds of treatment failure. In total, 101 patients with two years of follow-up were assessed. Compared with reimplantation at or before 18 weeks, TTR of >18 weeks was more strongly associated with treatment failure after controlling for American Society of Anesthesiologists status and prior revision surgery. Longer TTR was also associated with higher odds of unplanned readmission during the spacer period.

Study: Similar Patient Satisfaction after TKA with Customized versus Standard Implants

A study published online in Knee Surgery, Sports Traumatology, Arthroscopy found similar patient satisfaction between customized individually made (CIM) total knee arthroplasty (TKA) and off-the-shelf (OTS) TKA. Seventy-four CIM and 169 OTS TKAs were prospectively enrolled. Patient satisfaction at one year was 86 percent with CIM and 87 percent with OTS TKA. Most patients in both groups achieved a minimal clinically important difference in functional assessment scores. Postoperative Knee Society Scores for knee stability were significantly higher for CIM TKA.

Study: Greater Carbon Footprint with Endoscopic versus Open CTR

Endoscopic carpal tunnel release (CTR) produces a greater carbon footprint than open CTR, primarily due to facility-related and central processing-related factors, according to a study published online in the Journal of Hand Surgery. The researchers used a life cycle assessment to estimate the mass of carbon dioxide released over the course of each procedure using surgical data from 14 endoscopic and 14 open CTRs. Endoscopic CTR released 106.5 kg carbon dioxide equivalents (CO2-eq), which was significantly greater than that of open CTR (59.6 kg CO2-eq).

Study: Rise in Musculoskeletal Firearm Injuries during the COVID-19 Pandemic

A study published in the Journal of Bone & Joint Surgery reported a 63 percent increase in daily orthopaedic consultations for traumatic firearm injuries at a single center during the early months of the COVID-19 pandemic (March through September 2020). In total, 552 visits were identified from a prospective database, and 2020 data were compared with identical dates in 2017 and 2019. Average daily consultations rose from 0.62 before 2020 to 1.01 during the pandemic. Patients presenting during the pandemic were more likely to be uninsured, which was significantly associated with a lower odds of outpatient follow-up.

Diversity Town Hall: AAOS Reports Progress, Identifies Room for Continued Improvement

In the virtual AAOS Diversity Town Hall, Anthony Johnson, MD, FAAOS, chair of the AAOS Diversity Advisory Board, presented the membership with a status update of AAOS’ efforts to expand diversity and inclusiveness within the organization and the profession. Dr. Johnson, along with three members of the AAOS Presidential Line also highlighted what is on the horizon.
