Study: Greater Dislocation Risk with Posterior Approach than Direct Anterior Approach THA

A retrospective, single-institution study of 8,840 total hip arthroplasties (THAs), published online in The Journal of Arthroplasty, found a 4.9 times higher risk of dislocation associated with the posterior approach (PA) compared with the direct anterior approach (DAA). Dislocation rates were significantly lower for DAA versus PA approach (0.5 percent and 3.3 percent). Five-year survivorship was also greater in the DAA cohort (99.1 percent versus 95.4 percent).

Study: Nonunion Rates with Retrograde Nailing or Lateral Locked Plating for Distal Femur Fractures

A retrospective study of 106 patients, published online in Injury, reported a lower nonunion rate associated with retrograde intramedullary nailing (rIMN; n = 50) than lateral locked plating (LLP; n = 59) for treating complete articular distal femur fractures. Average time to union in the rIMN group was 6.0 months, compared to 6.6 months with LLP. Nonunion rates were significantly reduced in the rIMN group versus the LLP group (11.8 percent versus 27.5 percent, respectively). Eight secondary procedures for nonunion occurred in the rIMN group, compared with 18 procedures in the LLP group, though this difference was not statistically significant.

Study: Higher Revision after Mobile- versus Fixed-Bearing Total Ankle Arthroplasty

A study of 302 patients, published in the Journal of Bone & Joint Surgery, reported a three-fold higher rate of revision with mobile-bearing versus fixed-bearing implants at three-year follow-up after total ankle arthroplasty. The mobile-bearing group had a significantly elevated hazard ratio for component revision relative to the fixed-bearing group (hazard ratio = 3.31). Thirty-one reoperations occurred in the mobile-bearing group, compared with 14 in the fixed-bearing group. The authors noted that reoperations in the mobile-bearing group were more extensive than those in the fixed-bearing group.

Study: ALIF Wound Complications Are More Common among Patients in WHO Obesity Classes II and III

A retrospective study published online in The Spine Journal found a significantly elevated risk for postoperative wound complications after anterior lumbar interbody fusion (ALIF) in patients who are in the World Health Organization (WHO) classes II and III for obesity, compared with other obesity classes. The researchers queried the National Surgical Quality Improvement Program database to identify 10,934 cases from 2009 to 2019. Obesity classes II and III also had significantly higher likelihoods of superficial infection, organ space infection, and wound disruption compared to nonobese patients and patients with WHO class I obesity.
