Study: Risk Factors of Blood Transfusion after Total Joint Arthroplasty
Factors associated with blood transfusion after total joint arthroplasty include lower preoperative hematocrit, a history of bleeding disorders, and preoperative transfusion, according to a retrospective study published online in The Journal of Arthroplasty. A national database was queried for primary and revision total hip and total knee arthroplasties between 2011 and 2019. Overall transfusion rates decreased from 21.4 percent to 2.5 percent during the study period. Transfusion was also more frequent in patients who were older or female, had greater comorbidities, received nonspinal anesthesia, and had longer operative time.

Study: High Rate of Femoral or Acetabular Abnormalities among Patients with FAI
A systematic review published online in Knee Surgery, Sports Traumatology, Arthroscopy, found that up to half of patients presenting with symptomatic femoroacetabular impingement (FAI) display femoral abnormalities. Forty-three articles were evaluated (n = 8,861). “Normal” femoral or acetabular version in FAI was defined as 10 to 25 degrees. An estimated 31 percent and 51 percent of patients with FAI displayed abnormal central acetabular and femoral version, respectively.

| Study: Low-intensity Pulsed Ultrasound after Screw Fixation for Metatarsal Stress Fracture
A retrospective study found that low-intensity pulsed ultrasound (LIPUS) for bone healing after intramedullary screw fixation for proximal fifth metatarsal stress fractures did not significantly improve return to sport or radiographic bone union, compared to patients who did not receive LIPUS. These findings were published online in BMC Musculoskeletal Disorders. In total, 101 feet were assessed (LIPUS, n = 57). Mean time to return to sport was 13.7 weeks and 13.2 weeks in the LIPUS versus non-LIPUS groups. Mean time to radiographic bone union was 11.96 weeks and 12.0 weeks, respectively.

| COVID-19 Vaccination Requirements Expanded to Medicare- and Medicaid-certified Facilities
On Thursday, the Biden-Harris Administration announced that COVID-19 vaccinations will be required for staff in all Medicare- and Medicaid–certified facilities. Vaccinations were previously required for workers in nursing home facilities, but Thursday’s decision expands the mandate to require vaccines for staff at hospitals, ambulatory surgical settings, home health agencies, and more, for the facility to qualify for participation in the Medicare and Medicaid programs. The Centers for Medicare and Medicaid Services are expected to release an Interim Final Rule with Comment Period on this issue in October.

| HHS Announces Additional $25.5 Billion in COVID-19 Provider Funding
On Friday, the Department of Health and Human Services (HHS) announced $255 billion in new funding for healthcare providers affected by the COVID-19 pandemic, including funds for the Provider Relief Fund (PRF) Phase 4 and the American Rescue Plan for providers serving rural Medicaid, Children’s Health Insurance Program, or Medicare patients. HHS also announced a final 60-day grace period for PRF Reporting requirements for providers who do not meet the Sept. 30 deadline for the first Reporting Time Period. HHS will not initiate collection activities or similar enforcement actions for noncompliant providers during this grace period, but deadlines to use funds and the Reporting Time Period will not change.
