Study: Ten-year Outcomes in TKA after UKA versus TKA after HTO

A study published online in Knee Surgery, Sports Traumatology, Arthroscopy evaluated long-term survival and complication rates among patients who had undergone primary total knee arthroplasty (TKA; n = 315,071) or TKA following unicompartmental knee arthroplasty (UKA; n = 1,284) or high tibial osteotomy (HTO; n = 2,177). Revision among the UKA and HTO cohorts versus the primary TKA cohort were 4.2 percent versus 2.0 percent versus 2.1 percent. At 10-year follow-up, revision risk was significantly higher in the UKA and HTO cohorts. The overall rate of complications was comparable between groups.

Study: Improved Gait, Increased Sedentary Behavior at Two Years Post-THA

A study published in the July 7 issue of the Journal of Bone and Joint Surgery found significantly improved gait function two years after primary total hip arthroplasty (THA). Activity was measured via a wrist-worn accelerometer over a 24-hour period preoperatively and after one and two years. Despite gait improvements, patients showed more sedentary behaviors, suboptimal sleep, and decreased physical activity at two years compared to preoperative levels.

Study: Poor Outcomes of Posterior Open Wedge Glenoid Osteotomy for PSI with Glenoid Retroversion

In six patients (n = 7 shoulders) with posterior shoulder instability (PSI) with excessive glenoid retroversion, posterior open wedge glenoid osteotomy did not restore shoulder stability, recenter the joint, or prevent progression of osteoarthritis, according to a study published online in the Journal of Shoulder and Elbow Surgery. Mean follow-up was 15 years Six of seven shoulders (86 percent) showed recurrent, symptomatic PSI postoperatively, including one case of revision. All patients had continued glenoid arthritic progression.

Study: Thirty-day Infection after Fasciotomy for ACS Management

A retrospective study published online in Injury found 16.7 percent of fasciotomies (n = 370) for acute compartment syndrome (ACS) developed infection within 30 days of index procedure. Pseudomonas aeruginosa was the predominant organism, comprising 35 percent of overall infections. Burn and trauma patients were in the highest-risk groups for infection. Ten patients required amputation for infection control and one patient mortality occurred.

‘Technical Corrections’: CPT Evaluation and Management Publishes Additional Guideline Revisions

Changes to the CPT® Evaluation and Management (E/M) Office or Other Outpatient guidelines changes issued on Jan. 1 raised several questions about the ordering of tests and medical decision-making (MDM) credit. On March 9, the American Medical Association published “technical corrections” to clarify several crucial points, but physicians and specialty societies have argued that the revisions greatly impact the reporting of E/M services. This article analyzes some of these revisions and their implications for orthopaedic surgeons.
