First Vice President Felix H. Savoie III, MD, Speaks to the Value of AAOS Membership

At the Your Academy 2021 event during the Annual Meeting, AAOS First Vice President Felix H. (Buddy) Savoie III, MD, FAAOS (who will become president in 2022), explained that his priority as president will be “to make things easier for our AAOS members to take care of our patients.” He spoke to some of the bureaucratic and financial snares that interrupt patient care. “With leadership and support from [the Presidential Line], we have been striving to make prior authorization processes less rigorous and time-consuming and to get a relative value unit assigned to peer-to-peer reviews,” he said.

Study: Hyper- and Hyponatremia Predict Complications after Joint Arthroplasty

Hypernatremia and hyponatremia were significant predictors of complications in total joint arthroplasty, according to a study published online in The Journal of Arthroplasty. Researchers evaluated total hip (THA) and total knee arthroplasty cases between 2011 and 2017 via the National Surgical Quality Improvement. Hypernatremia and hyponatremia were both independent predictors of length of stay >2 days. Hypernatremia was predictive of ventilation >48 hours, unplanned intubation, Clostridium difficile infection, cardiac arrest, pneumonia, and mortality (in THA only). Hyponatremia was predictive for readmission, reoperation, surgical site infection, and transfusion.

Study: Subsequent Hip Fracture Occurs in One-quarter of Fracture Patients Aged >65 Years

A retrospective study published online in Osteoporosis International found that second hip fracture occurred in one quarter of patients aged >65 years who experienced any initial fracture and in one third of those who experienced index hip fracture. The researchers examined medical records of 115,776 patients via the ICES Data Repository. Median time to second hip fracture was around 1.5 years. One-year all-cause mortality was 25.9 percent after second hip fracture, compared to 15.9 percent in the entire cohort.

Randomized Study Finds Methylnaltrexone Does Not Improve GI Recovery after Spinal Arthrodesis

A randomized controlled study published online in The Spine Journal found that perioperative methylnaltrexone did not improve gastrointestinal (GI) recovery, opioid use, or pain scores after short-segment lumbar arthrodesis. Eighty-two patients were included in the study: 41 received subcutaneous methylnaltrexone and 41 received placebo. Postoperative GI outcomes, time to discharge, opioid consumption, and numeric rating scores for back, leg, or abdominal pain were similar between groups.

Report Finds Medicare Hospital Fund Is Still Projected to Become Insolvent by 2026

Medicare’s Boards of Trustees released their annual report to Congress on Tuesday and offered a similar forecast for the Medicare Hospital fund’s financial status as the 2020 report, which estimated the fund to become insolvent by 2026. The report found that the COVID-19 pandemic is not expected to significantly impact the fund’s financial status after 2024. The Boards did note, however, a decline in spending for care not related to COVID-19. The Social Security Trust Fund is anticipated to deplete by 2034, one year earlier than estimated in last years’ report.
