Many Strides Yet to Go: The Imperative to Increase Diversity in Orthopaedics

In a symposium yesterday, a panel of orthopaedic surgeons comprising mostly underrepresented minorities discussed racial disparities in orthopaedic surgery, from inequitable access to orthopaedic care to glaringly low numbers of racial and ethnic minorities in the profession. As moderator Toni M. McLaurin, MD, FAAOS, chief of orthopaedic service at Bellevue Hospital and director of diversity, equity, and inclusion at NYU Langone Orthopedics in New York City, stated, “The history of medicine in America is the history of racial disparities in American medicine.” The panelists, who included training program leaders, attendings, and fellows, related their personal experiences and shared ideas for improving diversity in the specialty.

PAC Luncheon Thanks Donors, Offers Analysis of Polarization in America

Members of the Orthopaedic Political Action Committee (OrthoPAC) attended the Donor Appreciation Luncheon yesterday to receive an update on the PAC’s standing and activities in 2021, as well as a political analysis from two-time Pulitzer Prize finalist Ronald (Ron) Brownstein. Guest speakers discussed the resiliency of the OrthoPAC during the COVID-19 pandemic, how it rose above numerous challenges at the start of this year, and the importance of political engagement from orthopaedic surgeons amid the deepening divide ahead of the 2022 election.

Study Gauges Weight-bearing CT to Diagnose Syndesmotic Instability

A study to be presented today at the Annual Meeting evaluated weight-bearing CT to diagnose syndesmotic instability in unilateral Weber B lateral malleolar ankle fractures. The results did not find widened medial clear space via radiograph yet demonstrated operatively confirmed syndesmotic instability. Syndesmotic joint volumetric measurements from the tibial plafond, fibular rotation, and middle tibiofibular distance were significantly increased with unilateral syndesmotic instability. Three-dimensional (3D) CT found larger ratios of distal tibiofibular articulation in patients with syndesmotic instability compared to non-3D measurements.

Study: Muscle Strength in Uninjured Leg after Autogenous Semitendinosus Graft

In a randomized controlled study of patients undergoing ACL reconstruction (ACLR) published online in Knee Surgery, Sports Traumatology, Arthroscopy, autogenous semitendinosus (ST) graft harvest from the uninjured leg showed favorable outcomes In total, 140 patients underwent either ipsilateral or contralateral ST graft harvest. ST graft harvest was associated with reduced isokinetic flexion strength (<10 percent) at six months, which resolved within 12 months Both patient groups showed overall muscle strength improvement compared to pre-injury, with comparable results.

Study: Lumbar Spinal Fusion Before Revision THA Is Linked to Dislocation and Re-revision

A retrospective study published in the Sept. 1 issue of the Journal of the AAOS ® found that lumbar spinal fusion (LSF) before revision total hip arthroplasty (THA) may increase risk of complications. The study included 328 patients with LSF prior to THA and 117 patients who underwent LSF after revision THA. At one year, dislocation rates were 6.7 percent versus 5.1 percent and re-revision rates were 8.5 percent versus 7.0 for fusion pre- and post-THA, respectively. LSF within one year after THA was associated with increased risk of dislocation or re-revision compared to longer intervals between LSF and THA.
