Study: Traction Forces of Postless versus Post-based Hip Distraction Systems after Hip Arthroscopy

In a prospective study published in Arthroscopy, postless traction systems significantly decreased the amount of traction force needed for adequate hip distraction for maximal and holding traction forces when compared with post-based systems. Intraoperative traction force and fluoroscopic distraction distance were measured in 105 post-based system patients and 51 postless patients to calculate hip stiffness coefficients at holding traction and maximal traction. The mean maximum traction force was 96.0 in the post-based cohort versus 69.9 in the postless cohort. Postless traction was found to be an independent predictor of decreased holding traction.

Study Finds No Increased Risk of Cyclops Lesion Formation after Remnant-preserving ACLR

A prospective study published in the Journal of Orthopaedic Surgery and Research compared the rate of range-of-motion restraints in remnant-preserving versus remnant-sacrificing ACL reconstruction (ACLR). The primary outcome was arthrolysis due to extension deficit or cyclops syndrome, and secondary outcomes included pain (NRS), patient satisfaction, and knee function (KOOS). Of the 164 patients included in the study, 60 received the remnant augmentation procedure. Between groups, there were no significant differences in NRS, KOOS, and patient satisfaction. Three cyclops lesion resections were performed in the non–remnant augmentation group, and one cyclops resection occurred in the remnant augmentation group.

Study Compares Open Procedure versus Arthroscopic Debridement for Chronic Medial Epicondylitis

In a retrospective study published in the Journal of Shoulder and Elbow Surgery, open procedure and arthroscopic debridement showed comparable outcomes for chronic medial epicondylitis. There were 38 patients in the open group and 19 patients in the arthroscopy group. The outcomes were operating time, visual analog scale (VAS), Disability of the Arm, Shoulder, and Hand (DASH) score, and complications. The mean operating time was 32.5 minutes in the arthroscopy group, compared with 23.5 minutes in the open group. VAS and DASH scores significantly improved in both groups. One incidence of transient ulnar neuropathy occurred in the open group.

Database Study: Management, Survival, and Mortality Rates of Appendicular Dedifferentiated Chondrosarcoma

A retrospective database study published in the Journal of Bone Oncology assessed risk factors for metastasis, and mortality and survival rates in patients with appendicular dedifferentiated chondrosarcoma. Demographics and clinical outcomes were analyzed in 205 patients. Of these, 51 patients presented metastasis at diagnosis, and the most common locations were in the lungs and bones. Female sex was found to be a protective factor for overall death, and metastases to sites other than the lungs had the highest risk of death. The combination of surgery and chemotherapy trended toward higher survival rates in nonmetastatic patients.

AAOS Board of Directors Approves CPG for the Management of ACL Injuries and Two AUC Reissues

The AAOS Board of Directors approved an update to the 2014 AAOS Clinical Practice Guideline (CPG) for the Management of Anterior Cruciate Ligament (ACL) Injuries and the reissues of two Appropriate Use Criteria (AUC) for the Management of Developmental Dysplasia of the Hip in Infants up to Six Months of Age: one intended for use by general pediatricians and referring physicians and the second for use by orthopaedic surgeons.
