Study: Favorable Outcomes of Nerve Radiofrequency Ablation for Knee OA

A systematic review published in the May 1 issue of the Journal of the AAOS ® assessed geniculate nerve radiofrequency ablation (RFA) for treating knee osteoarthritis (OA). Seven studies from 1966 to 2019 were included, comprising five high-quality and two moderate-quality randomized controlled trials. RFA had favorable outcomes across all studies. One high-quality and one moderate-quality study found statistically significant improvements in pain, function, and quality of life for patients who received RFA. RFA led to improvements in pain and functional scores compared to intra-articular corticosteroids and hyaluronic acid. There were no serious adverse events reported related to nerve RFA.

Study: Longevity of FVFG for Traumatic Osteonecrosis of the Femoral Head

A retrospective study published online in Injury identified outcomes of free vascularized fibular graft (FVFG) for treatment of patients with traumatic hip injuries resulting in osteonecrosis of the femoral head. The researchers evaluated 72 hips in 68 patients treated between 1980 and 2006. Mean follow-up was 11.6 years. Overall graft survival was 64.0 percent. For those without graft survival (36.0 percent), mean time to conversion to total hip arthroplasty (THA) was 8.4 years Stage at presentation was not associated with risk of THA conversion. Patients with surviving grafts had a mean improvement in Harris Hip Scores of 20.6 points.

Study Identifies Risk of Fall among Patients Scheduled for Elective Shoulder Surgery

A study published online in the Journal of Shoulder and Elbow Surgery evaluated risk of fall in patients scheduled to undergo either rotator cuff repair (RCR) or total shoulder arthroplasty (TSA). Gait speed and Timed Up and Go (TUG) testing was conducted in 198 patients prior to shoulder surgery (TSA, n = 80; RCR, n = 118). Overall, 59 percent of patients were at high risk for falls. More patients in the TSA group than the RCR group were considered at high fall risk (62 percent versus 38 percent). There were no associations found between age or ambulatory status and fall risk.

Study Compares Topical Skin Adhesives Versus Nylon Sutures for Incision Closure in Forefoot Surgery

A study published online in Foot & Ankle International compared topical skin adhesives versus interrupted nylon sutures for incision closure during forefoot surgery. In total, 70 patients (comprising 84 feet) who underwent hallux valgus scarf osteotomy or first metatarsophalangeal arthrodesis were randomized to the topical adhesive, 2-octyl-cyanoacrylate, or 3-0 nylon sutures. Topical adhesives had slower incision closure time compared to sutures (272 versus 229 seconds). At two weeks postoperatively, the adhesive group had worse Hollander Wound Evaluation Scale scores than the suture group (1.07 versus 0.60). Topical adhesives were associated with faster postoperative wound care time and less patient-reported pain compared to sutures.

Joseph A. Bosco III, MD, Looks Back on a ‘Year Unlike Any Other’

Immediate Past President Joseph A. Bosco III, MD, FAAOS, assumed AAOS presidency in March 2020, just as the COVID-19 pandemic blindsided the world and walloped the U.S. healthcare system. AAOS’ 2020 Annual Meeting had been canceled a few weeks prior, so instead of the traditional inaugural speech in front of a large crowd, Dr. Bosco spoke to the membership via a homemade video and declared that his sole focus was to chart a course that adhered to the “Mission, Vision, and Core Values” of AAOS. In this interview, Dr. Bosco looks back on his year at the helm and reiterates the theme he struck when he began it.
