Study Assesses Trends in 30-day Mortality after THA
A retrospective study published online in The Journal of Arthroplasty evaluated incidence and trends in 30-day mortality following elective total hip arthroplasty (THA). In total, the researchers identified 194,062 THA cases from between 2011 and 2018 from the American College of Surgeons National Surgical Quality Improvement Program database. Thirty-day mortality was 0.11 percent (n = 206). Mortality risk was highest among patients aged 80–89 years (0.352 percent) compared to younger patients. Patients with American Society of Anesthesiologists (ASA) physical status class IV, Charlson Comorbidity Index score of 4, and non-osteoarthritis diagnoses were associated with higher risk of mortality.

Study: High Rate of Humeral Loosening after TSA with Novel Short Humeral Stem
A retrospective study published online in the Journal of Shoulder & Elbow Surgery assessed whether a novel short humeral stem impacts the rate of humeral stem aseptic loosening after anatomic total shoulder arthroplasty (TSA). The researchers evaluated 184 shoulders receiving TSA performed with a grit-blasted, rectangular short humeral stem without ingrowth coating. Mean follow-up was 1.5 years, and 64.7 percent of patients had one-year radiographic follow-up. The overall rate of symptomatic aseptic humeral loosening was 10.9 percent (n = 20), and 54 percent of patients underwent revision TSA for humeral loosening. Three cases of probable or possible periprosthetic infection after revision occurred.

| Study: Fracture Location Associated with Opioid Requirements after Upper Extremity Fracture Fixation
A study published online in Injury measured how fracture location impacts perioperative demand after upper extremity fracture surgery. Using a national database, the researchers assessed 333,493 patients who underwent fixation of fractures of the clavicle through the distal radius between 2010 and 2020. Fixation of elbow, distal humerus, humeral shaft, and proximal humerus fracture was associated with significant increases in opioid filling (33.5 to 63.4 additional oxycodone 5 mg equivalents) and number of filled prescriptions (0.33 to 0.92) within one year, compared to distal radius fracture fixation and other types of injuries.

| Study: Impact of Bone Marrow Lesions on Osteochondral Allograft Transplantation Outcomes
A study published online in Arthroscopy investigated how preoperative bone marrow lesion (BML) size and location affect outcomes and complications after isolated osteochondral allograft (OCA) transplantation. Seventy-seven consecutive patients treated from 2009–2018 with preoperative MRI were included, with 82 percent presenting with a BML. Mean follow-up was 39.46 months. Graft failure occurred in 5 of 65 patients. BMLs were not significantly associated with survival. Preoperative Costa-Paz et al. classification was significantly positively correlated with postoperative functional and pain scores. One of two graders found that Welsch et al. classification was associated with greater risk of graft failure.

| Senate Votes to Appoint Chiquita Brooks-LaSure as CMS Head
On Tuesday, the Senate confirmed Chiquita Brooks-LaSure as the administrator of the Centers for Medicare & Medicaid Services (CMS) by a vote of 55-44. Ms. Brooks-LaSure previously served as deputy director for policy at the Center for Consumer Information and Insurance Oversight during President Obama’s administration, as well as director of coverage policy in the Department of Health and Human Services. Ms. Brooks-LaSure is the first Black woman to be head of CMS and will be responsible for oversight of the Medicare and Medicaid programs and exchanges set by the Affordable Care Act.
