Systematic Review: Extended Prophylactic Antibiotics for Total Joint Arthroplasty

A systematic review published in the June 1 issue of the Journal of the AAOS ® found “conflicting” evidence regarding the benefit of extended (>24 hours) antibiotics after total joint arthroplasty. Eleven studies were included, all of which evaluated a cephalosporin or penicillin antibiotic. Five studies evaluated IV antibiotics, one evaluated oral antibiotics, and the remaining studies included both delivery methods. Four studies reported a statistically significant decrease in periprosthetic joint infection with extended antibiotics compared with a control group, and seven studies found no significant differences with controls.

Study: Posteromedial Vertical Capsulotomy for Extension Gap Adjustment in Cruciate-retaining TKA

A study published online in Knee Surgery, Sports Traumatology, Arthroscopy reported that a posteromedial vertical capsulotomy (PMVC) was an effective option for intraoperative gap adjustment in cruciate-retaining total knee arthroplasty (TKA). In total, 120 knees treated for varus osteoarthritis were included, and 65 knees underwent PMVC. In the PMVC group, the mean extension gap significantly increased by 2.4 mm, which was 2.0 mm larger than the increase in the flexion gap. The researchers noted a preoperative extension range of motion cutoff of -10 degrees for predicting PMVC.

Randomized Study of Topical CBD for Arthritis-related Pain of the Thumb Basal Joint

A randomized controlled trial published online in The Journal of Hand Surgery found that topical cannabidiol (CBD) treatment significantly improved thumb basal joint arthritis-related pain, without adverse events. After a successful phase 1 skin test, eighteen patients were randomized to two weeks of twice-daily topical CBD with shea butter or shea butter alone, followed by a one-week “washout” period and then two weeks of crossover with the alternate treatment. CBD treatment led to improvements in patient-reported pain and functional scores compared with the respective control arm. Physical parameters (i.e., range of motion, grip, and pinch strength) were similar between groups.

Study: Relationship between Herniated Disk Position and Patient Pain, Disability

In patients with disk herniation, the size or position of the prolapsed intravertebral disk was not directly correlated to the patient’s pain and disability, according to a study published online in The Bone & Joint Journal. Fifty-six patients with disk herniation underwent MRI and, on the same day, completed the Visual Analog Scale and Oswestry Disability Index. There were 13 central prolapses and 43 lateral recess prolapses. The prolapse position was not associated with either patient-reported outcome measure.

Annual Social Security and Medicare Trustees Reports Published for 2022

On June 2, the Medicare Boards of Trustees published the annual reports on the Social Security and Medicare programs, and this year’s projections have improved compared with the 2021 reports. For example, the Old-Age and Survivors Insurance Trust Fund, which pays retirement and survivors benefits, will be able to pay scheduled benefits until 2034, (one year more than 2021’s projection). The Hospital Insurance Trust Fund, or Medicare Part A, which pays for services such as inpatient hospital care, will be able to pay scheduled benefits until 2028, two years later than previous projections.

Data Show Posterior Malleolus Repair of Unstable Rotational Ankle Fractures Leads to High Healing Rate

A study presented at the AAOS 2022 Annual Meeting found that direct repair of the posterior malleolus in the prone position in patients with unstable rotational ankle fractures was associated with a high rate of healing and no incidences of readmissions or deep infections. Seventy-seven consecutive patients with unstable ankle fractures underwent ORIF (open reduction-internal fixation). Mean follow-up was 13.7 months. All patients experienced a healed ankle fracture by a mean of 2.9 ± 1.1 months postoperatively. Eleven percent had a postoperative noninfectious wound complication, 5.2 percent had dysesthesia in the sural nerve distribution, and one patient (1.3 percent) had minor loss of the medial malleolus reduction.
