Study: Medicaid Status Is Associated with Increased Pain, Opioid Use after THA

A retrospective study published in the July 15 issue of the Journal of the AAOS ® reported worse postoperative pain and greater opioid use in total hip arthroplasty (THA) patients receiving Medicaid. The researchers analyzed data on 5,845 THA patients, including 5.6 percent with Medicaid. The remaining patients had commercial insurance or Medicare. Medicaid patients consumed a total of 178 morphine milligram equivalents (MMEs), compared with 89 MMEs in non-Medicaid patients.

Systematic Review Finds UKA Is Safe and Effective in Patients Aged <60 Years

In patients aged <60 years, unicompartmental knee arthroplasty (UKA) for medial osteoarthritis is safe and leads to improved function, according to a systematic review published online in Knee Surgery, Sports Traumatology, Arthroscopy. Seventeen articles, comprising 2,083 UKAs, were included. Follow-up ranged from one to 15 years. There were significant improvements in Knee Society Scores from preoperative levels. At 10 years, mean implant survival ranged from 86 to 96.5 percent.

Study: No Benefit of Blood Flow Restriction in Preoperative Quadriceps Strengthening Prior to ACL Reconstruction

Adding two weeks of home-based blood flow restriction (BFR) to a quadriceps strength prehabilition program prior to ACL reconstruction did not significantly impact improvements in muscle strength, according to a study published online in Arthroscopy. Forty-five patients were randomized to a two-week prehabilitation program with or without BFR. Quadriceps peak force measurements all significantly improved following the program, but there were no differences between groups. No complications from the program occurred.

Study: Ulna Centralization during Reconstruction after Resection of Giant Cell Tumor

A study published online in The Journal of Hand Surgery found that centralization of the ulna with ulnocarpal arthrodesis was effective for wrist reconstruction after resection of a Campanacci grade 3 giant cell tumor of the distal radius. The study included 26 patients with a mean follow-up of 32.8 months. At four, five, and six months, 38.5 percent, 77.0 percent, and 96.0 percent of patients, respectively, achieved radiologic ulnocarpal junction union. The overall complications rate was 10.7 percent. No nonunions, infections, or local recurrences occurred.

Revision THA: Femoral Impaction Grafting in Conjunction with a Cemented Femoral Component Is a Viable Alternative

In this Orthopaedic Video Theater review, Neil P. Sheth, MD, FAAOS, FACS, discusses a video on how to address femoral bone loss in patients undergoing revision total hip arthroplasty (THA). “To address femoral bone loss in the setting of revision THA, modular or nonmodular titanium fluted stems have become the workhorse for reconstruction. However, the use of femoral impaction grafting in conjunction with a cemented femoral component remains a viable alternative with excellent long-term outcomes,” Dr. Sheth wrote.
