Study: Anxiety, Depression Impact Satisfaction after Foot and Ankle Surgery

A retrospective study published in the August 15 issue of the Journal of the AAOS ® found patients with depression and anxiety (D/A) reported less improvement and satisfaction after foot and ankle surgery compared to patients without D/A. Of 340 patients (including 271 with two-year follow-up), 20 percent had D/A symptoms. Patients with D/A reported greater preoperative expectations compared to patients without symptoms, despite comparable rates of fulfilled expectations at two years (0.86 versus 0.78, respectively). Foot and Ankle Outcome Scores increased overall; however, patients with D/A had lower subscores for symptoms, activity, and quality of life.

Study: Ultrasonography Tops MRI for Detecting Partial-thickness Subscapularis Tears

Compared to MRI, ultrasonography (US) achieved greater sensitivity for diagnosing subscapularis (SSC) tears, according to a study published online in Arthroscopy. In total, 437 shoulder arthroscopy patients underwent preoperative US and MRI. Results were compared to diagnosis via arthroscopy. SSC tears occurred in 157 patients, including 126 with partial-thickness tears. Sensitivity for US versus MRI was 77.7 percent and 49.7 percent, respectively. For partial-thickness tears, sensitivity, specificity, and accuracy were 73.8 percent, 88.7 percent, 84.4 percent for US and 38.1 percent, 86.5 percent, and 72.5 percent for MRI, respectively.

Twin Study Finds BMI Impacts Heritability of OA of the Hip or Knee

A study published in the July 21 issue of the Journal of Bone & Joint Surgery evaluated how BMI impacts genetic susceptibility to osteoarthritis (OA). Using a Swedish national registry, researchers retrospectively assessed 29,893 twin pairs who underwent primary total joint arthroplasty for OA. Heritability estimates were 65 percent and 57 percent of hip and knee replacements, respectively. Higher BMI moderated heritability in both sexes for hips and in men for knees. In contrast, higher BMI in women was associated with increased heritability: 37 percent (BMI, 20 kg/m 2) versus 87 percent (BMI, 35 kg/m 2).

Preoperative MRSA Screening Does Not Lower SSIs after Lumbar Fusion

A retrospective study published online in The Spine Journal found that preoperative nasal methicillin-resistant Staphylococcus aureus (MRSA) screening did not improve rates of surgical site infection (SSI) after primary lumbar fusion. Overall, 1,884 patients were included in the study (40.1 percent of whom underwent MRSA screening) MRSA screening was more frequent in patients with lower Elixhauser index scores. MRSA testing, mupirocin prescriptions, perioperative parenteral vancomycin use, and intrawound vancomycin powder use did not impact rates of incision and drainage (I&D). Higher BMI and blood product units transfused was associated with I&D risk.

APM Incentive Payment Details from CMS Are Now Available Online

In June, eligible clinicians who were Alternative Payment Model (APM) Qualifying Participants (QPs) based on their 2019 performance began receiving their 5 percent APM Incentive Payments for 2021 from the Centers for Medicare & Medicaid Services (CMS). Details on the amounts paid to individuals and organizations can now be viewed on the Quality Payment Program (QPP) website. CMS has issued a list of unpaid QPs for whom they have been unable to complete payments. QPs who have not begun to receive their payment should download the file from the public notice, check for their name, and verify their Medicare billing information by Nov. 1. Jo A. Hannafin, MD, Speaks about Implicit Bias in Orthopaedics

In the mid-1990s, Jo A. Hannafin, MD, PhD, FAAOS, and a practice partner founded the Hospital for Special Surgery Women’s Sports Medicine Center to provide care specific to the needs of female athletes of all ages. Given the status of women’s sports medicine at that time, they encountered pushback along the way. Dr. Hannafin related this story and tackled the larger question of inclusiveness in orthopaedic surgery in a talk at the joint virtual Specialty Day of the Arthroscopic Association of North America and the American Orthopaedic Society for Sports Medicine.
